Is it mandatory to have to have a medical waiver for Bankart surgery on the shoulder or can the Meps Ortho Dr pass you without one if everything is good?

I'm not a doctor so I really can't answer any medical questions. You'll just have to get an answer from the Dr at MEPS. I wouldn't trust any info you get online about medical waivers, most people are only guessing or telling you something based on their own experience, which may not apply to your situation.
